Good news! Dr. Bazin has returned to practice this summer. Please call to get a history form to complete and to set up an appointment for you/your child soon, as her schedule is filling quickly.
Please know:
We are following CDC guidelines for safety. Dr. Bazin and staff will wear masks at all times. We require patients and anyone entering the office to also wear a mask. We observe social distancing as much as possible. After your evaluations, Dr. Bazin will provide a 2 hour conference (at no charge) to explain treatment options and discuss insurance reimbursement.
*Dr. Bazin is not a provider for insurance companies, so please be prepared to pay at the time of service. Her staff will provide you with an insurance-ready claim form for you to file with your insurance provider for direct reimbursement.
*Dr. Bazin and her patient population need an artificial scent-free environment. So please do not wear heavy perfume or clothing laundered in strongly scented detergents.
